Wife has really found this helpful so far in easing her tummy and back pain during her twin pregnancy. The bottom baby does start kicking in protest when she sits or lies down with it tightly, so she usually loosens it a bit then, but particularly helps when standing/walking. She is usually a medium, and this size works, so no need to go a size up.
